Wednesday, January 2 Vestibular Disease and French Bulldogs Part Two Many adult dogs experience an episode that looks like a seizure or a stroke, and we are concerned that some Frenchies are being euthanized needlessly due to misdiagnosis. Vestibular disease presents suddenly. One minute your dog is running around or chewing a toy, and the next minute she can't stand up or remain standing, or she can't focus her eyes, and they are moving back and forth quickly in a motion called nystagmus. Her head may tilt either immediately or sometime later in the hours or days following the initial event. She may drool. She may panic. If you are like most people, you will almost certainly panic. Try to stay calm and take her to the vet as soon as you can. Deo suffered from vestibular disease. In this photo you can see his head tilt and his left eye is a little less bright because of facial paralysis on that side. Here's what is happening: your dog has lost her sense of balance. She can't figure out her relationship to the ground and that's why she's staggering. Most episodes are "idiopathic" which means nobody knows what causes them. Paradoxical vestibular disease refers to a distinct localization to the cerebellum: specifically, the caudal cerebellar peduncle or the flocculonodular lobe2 that result in slightly different clinical signs than typical peripheral or central vestibular disease. Postural reaction deficits are the most important indicator of central disease; and deficits in cranial nerve V to XII are the most likely. Below is a brief discussion of some of the more common diseases seen in clinical practice. It is important to note that an intact tympanic membrane does not rule out OMI; in fact, infection most commonly results from 1 a descending infection from otitis externa, 2 hematogenous spread of microorganisms or 3 extension of microorganisms from the pharynx via the auditory tube. Treatment is antibiotic therapy, ideally based on culture and sensitivity, and supportive care. Recognition and prompt treatment of this complication can result in a good prognosis for recovery. Idiopathic Vestibular Disease Idiopathic vestibular disease is reported to be the second most common cause of peripheral vestibular disease in the dog. There is typically a peracute onset with affected animals showing clinical improvement within a few days and near complete resolution within 3 to 4 weeks. Though the cause of idiopathic vestibular disease is unknown, there is consideration that the cause may be due to a viral infection of the vestibular nerve. Hypothyroidism Hypothyroidism has been implicated as a cause for both peripheral and central vestibular disease in dogs. The pathogenesis of hypothyroid-associated vestibular disease has been described as a component of polyneuropathy or as myxedematous disease causing nerve compression cranial nerve VIII and sometimes cranial nerve VII as they exit the skull through the internal acoustic meatus. Thyroid hormone supplementation typically results in improvement within a few months. Ototoxicity Peripheral vestibular signs may develop if the ear has been flushed with any routine cleaning solution if the tympanum is ruptured. Clinical signs of vestibular disease may develop rapidly as soon as 10 minutes. Within 3 days to 3 weeks, central compensation results in gradual improvement of clinical signs; the compensation is maximal after about 3 months. The head tilt, however, may still be obvious and permanent. Chlorhexidine solutions can significantly contribute to ototoxicity and should be avoided if the status of the tympanum is unknown. Aminoglycoside antibiotics parenteral or topical can affect the hair cells, flocculonodular lobe and fastigial nucleus; streptomycin tends to have vestibular effects, whereas neomycin, gentamycin, kanamycin, and vancomycin tend to have more effect on the auditory system. In addition, loop diuretics, cisplatin and propylene glycol have also been implicated as potentially ototoxic. Female, small breed dogs 4 to 8 years of age are predisposed, and diagnosis is based on MRI and, cerebrospinal fluid analysis, with or without infectious disease testing. Treatment involves immune suppression with glucocorticoids plus a chemotherapy drug cytarabine arabinoside, procarbazine or immunomodulating drug cyclosporine, leflunomide. Aggressive long-term treatment is necessary, and the prognosis is guarded. Diagnosis is based on history and clinical signs. Treatment includes cessation of metronidazole therapy and supportive care. It has been shown that diazepam administration 0. A careful history should be taken, including rate and duration of onset, whether the signs have progressed since onset, medications administered, diet, travel history, and existence of any other comorbidities. Symptomatic treatment is often beneficial as many patients that have vestibular disease experience varied degrees of severity of clinical signs including nausea, anorexia, anxiety, recumbency, circling, falling and rolling. For dogs that are mildly affected, meclizine, an antiemetic and antivertigo drug, is indicated. Other antiemetics such as maropitant, ondansetron or chlorpromazine can also be considered. Most of them are basically "outie" belly buttons. If small, they may seal up on their own and generally don't cause any problems. Larger ones may require surgery to close up the abdominal wall and prevent intestines from slipping out or getting twisted. Some dog breeds such as Weimaraners are predisposed to these hernias. Ideally dogs with anything other than very small umbilical hernias should not be bred. This is especially true if most of the puppies in the litter have one, suggesting genetic predisposition. What Exactly Is an Umbilical Hernia? In an unborn puppy, the umbilicus slips out through an opening in the puppy's stomach wall to connect to the placenta. When a puppy is born, his dam chews or breaks this umbilical cord or the breeder cuts it and ties it off. It dries and shrivels up, leaving behind the "belly button. Sometimes a small bit of fat may get stuck in the opening, which leaves an "outie" belly button. Occasionally the wall of the abdomen simply does not close all the way. That is when we say a puppy or dog has an umbilical hernia. Signs Your Dog Has an Umbilical Hernia If your dog has an umbilical hernia, when your dog lies on his back, you can feel a small opening at the umbilicus site. If it is the size of the end of your pinkie finger at 8 weeks of age, it is usually not a problem. Many of these small hernias will close up on their own by 6 months of age. If the hernia bubbles out or is bigger than a pinkie finger, there is a chance that intestines may be slipping in and out of the opening. At some point, a section of intestines could slide out through the hernia opening and get trapped or twisted. At that point, your dog needs emergency surgery. Signs you see with trapped intestines can include pain, your dog standing stiffly with his back arched, and possibly not eating or vomiting. If you see these symptoms, you need to call your veterinarian right away. Larger hernias will require surgery to close up the opening, leaving the intestines safely inside. If your puppy is not having any problems, your vet may recommend keeping an eye on it and doing the surgery at the same time as spaying or neutering. That means one bout of anesthesia and recovery for your pet and less cost for you. If intestines become trapped at any point, immediate emergency surgery is necessary. Some people have tried to encourage closure of hernias by using a belly wrap on their puppies with hernias. These result from the muscle failing to close over the area where the umbilical cord entered the body. Less frequently, hernias are found in the groin inguinal or next to the bottom perineal. The rarest kind of hernias involve the diaphragm and can be more difficult to diagnose as they are less obvious from outside. How are hernias diagnosed in puppies? Most external hernias are fairly easy to diagnose just by their location and feel. A soft swelling under the skin in one of the characteristic hernia locations is a strong indicator but your vet will check for an underlying hole in the muscle wall to be sure. These checks are quick, pain-free and harmless. In a small number of cases, or when an internal hernia is suspected, additional tests such as an ultrasound or X-ray may be necessary to diagnose the problem and assess its severity. These are straightforward, non-invasive procedures but may require your puppy to be sedated; your vet will be happy to discuss any questions you may have about this. Are there any problems hernias cause? How serious a hernia is depends on its location, its size and its contents. While many hernias cause few problems, some can make a pup very unwell. Most hernias, especially small ones, contain just fat which usually causes little problem apart from, possibly, some mild discomfort. The main concern with small hernias is that they get larger over time. Bigger hernias may allow a loop of the intestines or other organs such as the liver or bladder to pop out of the tummy. Now strangulated is a real medical term that means what it sounds like-that the blood supply to this area of intestine has been choked off. This is bad. If it were still small and staying that way, waiting would be fine. We do this type of surgery on small fry often. We use small instruments and suture. On a little tyke like this we would put some liquid food in by stomach tube under anesthesia and provide extra fluid under the skin. This would prevent low blood sugar problems. It is still young It is common for German Shepherds to go through a more nervous phase when they are puppies. If your German Shepherd is still a puppy then it is likely that it will improve over time. However, now would be a good time to teach it not to be nervous. A good way to do so would be to use positive reinforcement training where you reward it when it shows signs of not being nervous or it does things where it is not nervous and it normally would be.
